While reading a bible commentary (from the Nazarene Church) on the passage in Rev 5:6 about bowls of incense that were the prayers of the saints - the commentary said that modern churches using incense, believe it comes from this passage - but there is "no support" for such use in the first 300 years from the church fathers.
Is this correct?

Lack of reference doesn't mean lack of practice. Sometimes it means the use was so common it is taken for granted.
Here's an article from Catholic Answers on the use of incense in worship.
